Metro Home Health Care, a not-for-profit organization, dedicated to delivering accessible, high-quality health services in a focused caring and teaching environment while providing health advocacy for the community and people we serve.

We understand how important it is to have someone you can depend on when you want to stay at home as healthy as you can be. With hospital stays becoming shorter and an emphasis on outpatient services, we have built a home health agency that provides a cost effective way to help you or your loved one heal and live comfortably while maintaining dignity and quality of life. At Metro Home Health Care we bridge the gap between hospital or nursing facility and where the patient prefers to be – in the comfort of their own homes. The goal is for the patient to attain independence and remain in the setting they find most comfortable.

Many technological medical advances allow care previously available only in an institution to be delivered in the patient's home. Our services are delivered in your home by a team of specially trained health care professionals that are thoroughly screened and receive continuing education to uphold the valued expertise in their fields. We work closely with your physician to coordinate all aspects of your care, focusing on helping you recover and regain your independence through treatment and education according to your individual needs.

Culture of Safety and Quality: We strive to create a culture of safety and quality by developing a code of conduct, providing education, encouraging open communication, encouraging leaders to provide a team approach to safety and quality initiatives, providing leadership that defines how patients, family/caregivers, visitors, and other members of the community can help identify and manage issues of safety and quality, and implementing changes identified by the annual organization evaluation in order to maintain the culture of safety and quality.

Our Care Team

Metro Home Health Care provides intermittent care by a team of licensed and certified health care professionals, based on the clinical needs of the patient within a physician-authorized plan of care. Intermittent Care brings the best possible care to your home, whether you're coping with an illness, need help after hospitalization, or find it difficult to leave home on your own. We are dedicated to educating and empowering patients to take charge of their own health. Our services are guided by the belief that providing patients with more and better information leads to greater independence, quicker recovery, a higher level of satisfaction and reduced incidence of hospitalization.

Our care team is dedicated to providing quality patient care with a personal touch. During our home visits, we not only perform the prescribed treatments, but we also closely monitor any changes in your condition. Just as importantly, we take the time to listen to your concerns and respond to any questions or special needs that you or your family might have.
